Tom Madsen Airport sits on Amaknak Island in the Aleutian Islands, about a mile from the town of Unalaska. Anchorage is roughly 800 miles to the northeast, and the airport is the air gateway for the Dutch Harbor fishing port. The State of Alaska renamed it in 2002 for Charles Thomas Madsen Sr., a bush pilot known as the "Aleutian Aviator". It is also listed as Unalaska Airport, and its ICAO code is PADU.

The airport has a single asphalt runway, 13/31, measuring 4,500 by 100 feet. Airline operations are restricted: aircraft with more than 30 passenger seats need prior written permission. The airport office is staffed from 0800 to 1630.

Aleutian Airways flies daily between Dutch Harbor and Anchorage, and Grant Aviation links the airport with nearby Aleutian communities such as Akutan, Atka and Nikolski. Alaska Central Express carries cargo. The airport also has a restaurant and vehicle rentals. About 20,500 passengers boarded flights here in 2025.
